$ 2,269.60 Requirements Full-time position with 40 guaranteed hours per week Monday-Friday schedule (8:00 AM-4:30 PM) No weekends No holiday shifts Skilled Nursing Facility setting Collaborative interdisciplinary rehabilitation team Patient-centered care with opportunities to build meaningful relationships Supportive work environment focused on clinical excellence Responsibilities Evaluate patients and develop individualized physical therapy treatment plans based on functional goals and clinical findings. Provide evidence-based therapy interventions to improve strength, balance, mobility, endurance, gait, and overall functional independence. Perform comprehensive assessments and establish measurable rehabilitation goals. Monitor patient progress and adjust treatment plans as needed to maximize outcomes. Educate patients, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home exercise programs. Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals to coordinate patient care. Accurately document evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in compliance with facility and regulatory standards. Recommend and train patients in the proper use of assistive devices, adaptive equipment, and mobility aids. Ensure compliance with Medicare guidelines, infection prevention protocols, and all applicable regulatory requirements. Promote patient safety while delivering compassionate, high-quality rehabilitation services. Maintain productivity standards while providing exceptional patient-centered care. Qualifications Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of practice (or eligibility for licensure). Prior experience working in a Skilled Nursing Facility (SNF) is required. Strong clinical assessment, treatment planning, and documentation skills. Excellent communication and interpersonal abilities. Ability to work independently and collaboratively within an interdisciplinary healthcare team.
