C042416 Position Summary ND Global Consulting Services, Inc. is seeking a Physical Therapist to provide full-time physical therapy services to veterans and eligible dependents in a skilled nursing and rehabilitation setting. The Physical Therapist will support residents' mobility, strength, safety, positioning, functional independence, and discharge goals. Key Responsibilities Evaluate residents and develop individualized physical therapy plans of care. Provide therapeutic exercise, neuromuscular procedures, manual techniques, and approved physical agents to address mobility limitations and functional deficits. Support range-of-motion, ambulation, bed mobility, transfers, wheelchair, seating, positioning, and power-wheelchair programs. Participate in care-plan meetings, discharge planning, wound rounds, environmental/home assessments, and equipment recommendations. Maintain timely and accurate documentation of evaluations, treatments, progress, resident response, and changes in condition. Communicate effectively with the interdisciplinary care team. Supervise Physical Therapist Assistants and provide clinical direction in accordance with New York State law and facility policy. Maintain resident confidentiality and comply with facility documentation requirements. Collaborate with nursing, medical staff, social services, residents, families, the VA, and outside equipment vendors. Minimum Qualifications Current, unrestricted New York State Physical Therapist (PT) license . Minimum 3 years of physical therapy experience . Experience in skilled nursing, long-term care, geriatric rehabilitation, wheelchair/seating, or memory care is preferred. Must complete facility credentialing before starting, including proof of license/credentials, current physical examination, and TB/PPD documentation.
