Crouse Community Center is seeking a dedicated and compassionate Per Diem Physical Therapist to join our rehabilitation team. This role is ideal for a PT who values flexibility while still contributing meaningfully to resident care in a supportive, well-coordinated long-term care environment. As a Per Diem PT, you will provide high-quality physical therapy services that help residents maintain or improve functional mobility, independence, and overall well-being. You'll work collaboratively with nursing, medical staff, and interdisciplinary teams to ensure each resident receives individualized, person-centered care. What You'll Do Deliver resident-centered PT services including evaluation, treatment planning, and therapeutic interventions. Participate in care planning and collaborate with nursing, medical staff, and other disciplines. Document assessments and progress in accordance with regulatory and facility standards. Ensure safe, effective use of therapy equipment and maintain a clean, organized therapy environment. Support MDS and interdisciplinary processes through timely communication and participation in reviews. Promote resident safety during all therapy sessions. Requirements:

What We're Looking For Current NYS Physical Therapy license in good standing. Bachelor's or Doctorate in Physical Therapy from an accredited program. Experience in long-term care or a similar healthcare setting preferred. Strong communication skills and a commitment to high-quality resident care. Ability to work independently and collaboratively within a multidisciplinary team. Professional, positive, and resident-focused demeanor.
