Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will play a vital part in helping patients regain mobility, reduce pain, and improve their overall quality of life through personalized treatment plans. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient hospitals, home health, and community care environments. Your expertise in physiotherapy and patient rehabilitation will ensure that each individual receives tailored interventions that promote recovery and independence. Responsibilities Assess patients' physical conditions by reviewing medical histories, conducting examinations, and evaluating mobility and functional abilities. Develop comprehensive treatment plans aligned with patient goals, clinical standards, and best practices in physiotherapy. Implement therapeutic interventions such as manual therapy, trigger point therapy, exercise programs, and modalities to facilitate recovery. Collaborate effectively with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate care. Document patient progress accurately using electronic health records (EHR) systems while adhering to clinical documentation standards. Educate patients and their families on injury prevention, home exercises, and community resources to support ongoing recovery. Manage discharge planning by preparing patients for transition to home or community settings with appropriate care instructions and follow-up plans. Skills Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, and geriatrics to inform treatment strategies. Proven experience working with diverse patient populations in outpatient clinics, inpatient hospitals, or home health settings. Excellent communication skills for engaging with patients of all ages and backgrounds effectively. Familiarity with outpatient rehabilitation center practices and outpatient experience is highly desirable. Ability to develop innovative treatment plans based on individual needs while maintaining compliance with clinical documentation standards. Proficiency in electronic health records (EHR) management systems and EMR systems for accurate recordkeeping. Knowledge of discharge planning processes and working within acute care environments to support patient recovery goals.
