Work Environment Located in the vibrant city of Syracuse, New York, our organization fosters a supportive and collaborative atmosphere where every team member plays a critical role in patient recovery. As a Physical Therapist in our home care setting, you will work both in the clinic and directly in patients' homes, utilizing a range of therapeutic modalities tailored to individual needs. Our practice emphasizes teamwork and consistency, allowing you to engage deeply with your patients in their recovery journeys. The typical work schedule is Monday through Friday, from 2 PM to 8 PM, allowing for a balanced work-life dynamic. Key Responsibilities Manage therapeutic care for a designated group of patients, collaborating closely with physicians and the broader patient care team. Conduct thorough patient assessments and develop individualized care plans that emphasize patient engagement, goal achievement, and overall positive experiences. Carry out evaluations and treatment interventions within your professional scope, always adhering to established laws and ethical guidelines. Continuously assess and evaluate patients' therapeutic, rehabilitative, and functional statuses to inform the development and adjustment of comprehensive care plans. Deliver skilled therapeutic services directly in patients' homes as necessary, ensuring they receive consistent and effective treatment. Provide comprehensive education to patients, families, and caregivers about therapeutic exercises, the use of modalities, and the operation of prosthetic devices. Prioritize patient caseloads, determine daily staffing requirements, and effectively coordinate assignments to optimize treatment delivery. Patient Population / Client Focus In this role, you will primarily focus on orthopedic patients, including individuals recovering from surgery, managing chronic pain, and sports-related injuries. With both adult and pediatric demographics, you will tailor your therapeutic approaches to meet the diverse needs of your patients, fostering a rehabilitation environment that promotes healing and functional recovery across all age groups. Qualifications Hold a valid license in Physical Therapy. Possess a Doctor of Physical Therapy (DPT) degree. Basic Cardiac Life Support (BCLS) certification is required, or must be completed within 30 days of employment. No prior experience is necessary, making this a great opportunity for both new graduates and seasoned professionals willing to join our vibrant team. Compensation and Benefits This rewarding position offers a competitive annual salary ranging from
USD 100,000
