With Freedom's home care team you'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. You'll provide home-based therapy care for patients. Interacting with patients' families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey. Home care provides context and real-world perspective about what will really help patients restore their health. PT Responsibilities Identify and meet patients' goals and needs Offer treatments that help improve clients' motion and mobility Develop care plans using a variety of treatment techniques Create fitness- and wellness-oriented programs tailored to patients' specific needs Provide quality, personalized and evidence-based care and proven interventions Motivate patients during treatment in order to help them function optimally Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination Consult and practice with other health professionals Evaluate and monitor effects of therapy and communicate progress Document patient care services Supervise PTA's PT Skills Ability to manage patients with different types of personalities Current knowledge of treatment practices Drive to continually learn and grow Professional behavior Excellent interpersonal communication skills Valid license to practice physical therapy in
