We are expanding the program offerings at our client community acute care hospital and need to add team members to an already-excellent rehab staff. We've achieved many things over our 18 year partnership with the acute care rehab hospital and are now developing more offerings to service the surrounding communities. We are in the early stages and want to add full time PTs to be a part of the development effort and to help our existing team handle the inpatient and outpatient caseload increases. • Plan, develop, organize, implement, evaluate, and direct physical therapy, as well as, its programs and activities, in accordance with current rules, regulations, and guidelines that govern the agency and the practice of physical therapy.
• Interview patients and families to determine type of treatment, schedule therapy and participate in care team meetings as appropriate.
• Perform administrative duties, such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary, including but not limited to maintaining treatment records, patient files, and progress notes as required.
• Participate in discharge planning, development and implementation of patient care plans, patient assessments, etc., as necessary.
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment with the patients, their families, medical and agency personnel.
• Review request for physical therapy and physicians' orders.
• Recommend modifications and changes in the patient's therapy program based on own evaluation of progress.
