The primary purpose of this position is to provide skilled physical therapy services to patients in their homes, including: Evaluating patient needs and developing individualized treatment plans. Delivering therapy interventions to improve mobility, function, and quality of life. Collaborating with physicians, nurses, and other members of the care team. Educating patients and families on safe exercises and techniques. Documenting care accurately in the electronic health record (EHR) system. Home health experience is preferred but not required—we are happy to provide training and support to help you grow in this specialty. Licensed PT with current OH state license.
