Job Description

Mercy is seeking a compassionate Physical Therapist to deliver evidence-based rehabilitation services within our Healthcare & Medical Services department. In this role, you will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to restore function, mobility, and quality of life. You'll collaborate with an interdisciplinary team, educate patients and families, and document outcomes using EMR systems. Responsibilities Evaluate patients using standardized assessments to determine functional limitations and rehabilitation needs Develop and implement individualized, evidence-based treatment plans and goals Provide therapeutic exercises, manual therapy, and modalities to improve mobility, strength, and pain management Educate patients and families on home exercise programs, safety, and condition management Collaborate with physicians, nurses, and other therapists to coordinate comprehensive patient care Document evaluations, daily notes, and progress reports in the EMR accurately and timely Monitor patient progress, adjust treatment plans, and participate in care conferences Ensure compliance with regulatory, ethical, and safety standards Mentor students or new staff as appropriate and contribute to team meetings Participate in continuing education and quality improvement initiatives Required Skills Patient evaluation and assessment Treatment planning and goal setting Therapeutic exercise prescription Manual therapy techniques Use of physical agents and modalities (e.g., ultrasound, e-stim) Gait training and balance rehabilitation Orthopedic and neurologic rehabilitation Post-surgical rehabilitation protocols Electronic Medical Record (EMR) documentation Patient and family education Interdisciplinary team collaboration Pain management strategies Functional outcome measurement tools (e.g., FOTO, FIM) Time management and caseload prioritization Knowledge of rehabilitation and privacy regulations (e.g., CMS, HIPAA)
