Minimum Requirements Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA Active State License is Required to Start the Assignment BLS Certification May Be Required from AHA or ARC Current unencumbered Oregon Physical Therapy license required. Healthcare Provider Level BLS required. Must be able to travel to multiple points of care.
