Job Description

Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to provide high-quality rehab services to geriatric and sub-acute residents in skilled nursing facilities. Working under the direction of a Physical Therapist, you will carry out individualized treatment plans, including therapeutic exercise, gait and balance training, and functional mobility activities to improve safety and independence. You'll collaborate closely with an interdisciplinary team, document progress, educate patients and families, and contribute to a supportive, patient-focused culture that values clinical excellence, integrity, and professional growth. Responsibilities Provide physical therapy interventions under the supervision of a licensed Physical Therapist Implement individualized treatment plans focused on mobility, strength, balance, and functional independence Assist geriatric and sub-acute residents with safe transfers, gait training, and therapeutic exercises Accurately document treatments, patient responses, and progress in the EMR system Educate patients, families, and caregivers on home exercises, safety, and functional strategies Collaborate with an interdisciplinary team, including nursing and rehab staff, to coordinate care Monitor patient safety, follow fall-prevention protocols, and report changes in condition Participate in care conferences and contribute to ongoing plan-of-care adjustments Support a compassionate, patient-focused culture consistent with company values Engage in professional development and follow regulatory and facility compliance standards Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Treatment documentation (EMR) Patient and family education Safety and fall-prevention techniques Interdisciplinary team collaboration Range of motion techniques Transfer and positioning skills Outcome measurement and reporting
