Position Summary We treat out Clinicians differently than other providers, Our Full Time Sallaried Team Members are considered an integral part of our team, and are members of the Leadership Team, actively providing feedback and engaging in helping us become the best home health provider in the area . In many cases, our Full Time Team members recieve a base sallary and then can achieve steps in pay based on the number of weekly visits completed...(restrictions and rules are laid out in indivual contracts) We are seeking a caring and motivated Home Health Physical Therapist to join our team. In this role, you will provide skilled physical therapy services to patients in their homes, helping them improve mobility, strength, safety, function, and overall independence. The ideal candidate is dependable, compassionate, organized, and comfortable working independently while collaborating with the broader care team. Responsibilities
- Perform in-home physical therapy evaluations, assessments, reassessments, and treatments in accordance with physician orders and the patient's plan of care.
- Develop and implement individualized treatment plans focused on improving mobility, balance, strength, endurance, safety, and functional independence.
- Educate patients and caregivers on home exercise programs, fall prevention, safe transfers, equipment use, and strategies to support independence at home.
- Monitor patient progress, update goals as appropriate, and communicate changes in condition to the care team and physician as needed.
- Complete accurate and timely clinical documentation in accordance with company standards and regulatory requirements.
- Collaborate with nurses, occupational therapists, speech therapists, aides, physicians, families, and other members of the interdisciplinary team.
- Supervise Physical Therapist Assistants, when applicable, in accordance with state regulations and company policy.
- Promote a positive patient experience through respectful, compassionate, and professional care. Qualifications
- Current Physical Therapist license in the state of practice.
- Graduate of an accredited Physical Therapy program.
- Current CPR/BLS certification preferred or required based on company policy.
- Home health experience preferred, but not required for the right candidate.
- Strong clinical judgment, communication skills, time management, and documentation skills.
- Ability to work independently in the home setting while maintaining regular communication with the care team.
- Reliable transportation, valid driver's license, and auto insurance as required for home visits.
- Commitment to delivering high-quality, patient-centered care. Schedule and Territory This is a Full Time position with flexible scheduling based on patient needs. Clinician's schedule thier own visits to enable them create and maintain thier own schedules. Visits are completed in patients' homes. Pay:
