Duties Assess patients' physical conditions and develop personalized treatment plans aligned with clinical best practices and patient goals Provide hands-on therapy interventions such as therapeutic exercises, trigger point therapy, and gait training to promote recovery and functional independence Conduct discharge planning to ensure smooth transitions between care settings and ongoing patient support Maintain accurate clinical documentation in compliance with healthcare standards and electronic health records (EHR) systems Collaborate effectively with physicians, nurses, caregivers, and other healthcare team members to coordinate care strategies Educate patients and their families about treatment procedures, home exercises, and safety measures for optimal rehabilitation outcomes Monitor patient progress regularly and adjust treatment plans as needed to maximize recovery potential Company Overview REHAB1 is a dedicated, therapist-owned private practice committed to delivering personalized, respectful care through one-on-one therapy sessions. Pay:

$50.00 - $55.00 per hour
