Physical Therapist (PT) Join the Team at William Penn Nursing and Rehab in Lewistown, Pennsylvania William Penn Nursing and Rehab, a 121-bed skilled nursing facility located in the heart of Lewistown, Pennsylvania, is seeking a part-time Physical Therapist (PT) to join our team. As a Physical Therapist at our facility, you will have the opportunity to work with a diverse patient population, providing compassionate and high-quality care in a supportive environment. About the
Role:
As a Physical Therapist at William Penn Nursing and Rehab, you will be responsible for: Evaluating and treating patients with a range of physical disabilities and conditions Developing and implementing individualized treatment plans to achieve patient goals Providing education and support to patients and their families Collaborating with other healthcare professionals to ensure seamless care Maintaining accurate records and reporting changes in patient condition
Responsibilities:
Conduct initial and ongoing assessments of patients' physical function and mobility Develop and implement treatment plans to address patients' physical needs Provide hands-on therapy interventions, including but not limited to exercise, stretching, strengthening, and gait training Monitor patients' progress and adjust treatment plans as needed Communicate effectively with patients, families, and healthcare teams to ensure coordinated care
