Physical Therapist (PT) - Full-Time Lock Haven Rehabilitation and Senior Living Join Our Growing Rehabilitation Team Lock Haven Rehabilitation and Senior Living is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our dynamic rehabilitation team. This position offers the opportunity to work collaboratively with therapists, nursing staff, physicians, residents, and families to deliver high-quality rehabilitative care that promotes mobility, independence, functional improvement, and enhanced quality of life. Responsibilities Evaluate residents and develop individualized treatment plans based on clinical findings, functional abilities, and resident goals. Provide skilled physical therapy interventions designed to improve mobility, strength, balance, coordination, and overall functional independence. Monitor resident progress and adjust treatment plans as needed to support positive outcomes. Accurately document evaluations, treatments, progress notes, and other required clinical information in a timely manner. Collaborate with interdisciplinary team members to support resident care goals and discharge planning. Educate residents, families, and caregivers regarding treatment plans, safety techniques, and strategies to maximize functional outcomes. Maintain compliance with facility policies, professional standards, state regulations, and federal guidelines. Participate in care conferences and team meetings as appropriate. Ensure a safe and therapeutic treatment environment for all residents. Qualifications Graduate of an accredited Physical Therapy program required. Current Physical Therapist license in the State of Pennsylvania in good standing. CPR certification preferred. Previous experience in skilled nursing, long-term care, rehabilitation, or post-acute settings preferred. Experience with Net Health documentation software preferred. Experience with PointClickCare (PCC) preferred. Strong clinical, communication, organization, and interpersonal skills. Ability to work effectively as part of an interdisciplinary team. Why Join Us?
