We are looking for a compassionate and flexible clinician who enjoys helping residents improve mobility, independence, and overall quality of life. Documentation systems: Net Health and PCC

Positive team-oriented atmosphere Responsibilities Evaluate and treat residents in a SNF/LTC setting

Develop individualized treatment plans and functional goals

Collaborate with therapy, nursing, and interdisciplinary teams

Complete timely and accurate documentation using Net Health and PCC

Support resident safety, mobility, and independence Qualifications Current Pennsylvania PT license

SNF/LTC experience preferred, but not required

Strong communication and clinical skills

Flexible and dependable
