Develop individualized plans of care based on physician referrals, patient goals, and clinical findings. Modify treatment plans as patients progress and their needs change. Help patients improve mobility, strength, balance, flexibility, endurance, and overall function. Collaborate with Physical Therapist Assistants (PTAs) to ensure continuity and effectiveness of care. Collaborate with the Care Team Communicate patient progress, recommendations, and treatment outcomes with physicians, caregivers, and interdisciplinary team members. Provide education and home exercise programs to patients and families to support continued success beyond therapy sessions. Serve as a resource and advocate for patients throughout their rehabilitation journey. Maintain Clinical Excellence Complete accurate and timely documentation, including evaluations, progress notes, discharge summaries, and treatment records. Ensure compliance with regulatory, organizational, and department standards. Complete charge capture and required statistical reporting accurately.
