Overview We are seeking a qualified and compassionate Physical Therapist - PRN to join our dynamic healthcare team. The ideal candidate will possess a strong background in physiotherapy and demonstrate expertise in patient rehabilitation across diverse settings. This role offers flexibility and the opportunity to make a meaningful difference in patients' lives by delivering personalized therapy services within outpatient clinics, hospitals, and home health environments. Duties Assess patients' physical conditions and develop individualized treatment plans to promote recovery and improve mobility. Implement therapeutic interventions including manual therapy, trigger point therapy, and exercise programs tailored to patient needs. Collaborate effectively with healthcare professionals such as physicians, nurses, and other therapists to coordinate patient care. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) systems. Conduct discharge planning to ensure seamless transition of care from inpatient to outpatient or community settings. Work with geriatric patients, pediatrics, neurology cases, and orthopedics to address diverse rehabilitation needs. Educate patients and their families on treatment procedures, home exercises, and community resources to support ongoing recovery. Experience Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health settings. Demonstrated expertise in developing care plans for patients with orthopedic, neurological, or geriatric conditions. Familiarity with outpatient rehabilitation center operations and inpatient hospital environments. Knowledge of physiology, anatomy, medical terminology, and clinical documentation standards essential for effective patient care. Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation management. Prior involvement in discharge planning and working with multidisciplinary teams to optimize patient outcomes. Strong client communication skills coupled with the ability to work independently while adhering to clinical protocols.
