Role:

Licensed Physical Therapist delivering personalized, in-home and senior living care to older adults.

Responsibilities:

Conduct evaluations, develop treatment plans, provide evidence-based therapy, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.

Qualifications & Skills:

Valid PT license, degree from an accredited program, basic computer and EMR proficiency, current CPR certification, and ability to meet physical demands including lifting, transferring, and driving.

Key Duties:

Support patient mobility, independence, and wellness through one-on-one care, promote safety, and foster patient relationships in various settings such as private residences and senior communities.
