Job Description
We are looking for a licensed Physical Therapist (PT) for PRN/per diem home health visits in the Los Angeles area. This opportunity is ideal for PTs looking for flexibility, additional visits, and the ability to choose their preferred coverage area.
Responsibilities:
Complete PT evaluations, reassessments, and discharge visits Provide therapy in patients' homes Develop and follow the plan of care Complete documentation in the agency EMR Communicate with the clinical teamRequirements:
Active California PT license Current BLS/CPR Active NPI Valid driver's license, reliable transportation, and auto insurance Home health experience preferred We offer flexible scheduling, per-visit pay, and the ability to choose your preferred coverage area.Pay:
