Job Description

Overview Join our dynamic healthcare team as a dedicated Physical Therapist (PT) or Occupational Therapist (OT), where your expertise will make a meaningful difference in patients' lives. In this role, you will provide comprehensive therapy services across diverse settings, including inpatient, outpatient, home health, skilled nursing facilities, and schools. Your passion for patient-centered care and evidence-based practice will drive positive outcomes and foster independence for individuals of all ages and abilities. Duties Assess patients' physical and occupational needs through detailed evaluations, including occupational therapy assessments and treatment plan development. Develop and implement personalized therapy programs aimed at improving mobility, daily living activities (ADLs), and functional independence. Collaborate with interdisciplinary teams to coordinate care plans, discharge planning, and case management strategies that align with patient goals. Assist patients with neurodevelopmental disorders, autism spectrum disorders support, pediatrics, geriatrics, and occupational health cases to optimize recovery and quality of life. Document clinical interventions accurately using electronic health records (EHR), ensuring compliance with medical documentation standards and medical terminology. Educate patients and their families on therapy techniques, adaptive equipment use, and strategies for ongoing self-care at home or in educational settings. Participate in clinical interprofessional collaboration to enhance treatment efficacy across inpatient, outpatient, school, or home health environments. Skills Strong knowledge of anatomy, physiology, and medical terminology relevant to physical rehabilitation occupational therapy. Proven experience working with individuals with disabilities, neurodevelopmental disorders, autism spectrum disorders support, or special education needs. Skilled in assisting with activities of daily living (ADLs), discharge planning, case management, and implementing occupational therapy programs. Ability to perform comprehensive occupational therapy assessments and develop effective treatment plans tailored to each patient's unique needs. Excellent client communication skills to foster trust and motivate patients throughout their recovery journey. Experience working in various settings such as inpatient units (including ICU), outpatient clinics, skilled nursing facilities, post-acute care environments, home health services, or schools. Familiarity with electronic health records (EHR) management for clinical documentation and progress tracking. Knowledge of pediatric occupational therapy techniques and experience working with students with disabilities or special education needs is highly valued.
