Prior home health experience/HCHB The Physical Therapist provides direct and indirect patient care services to individuals in their home or alternative settings, following organizational policies and physician-approved plans of care. Responsibilities include evaluating patient needs, developing and implementing individualized treatment programs, and administering complex therapeutic exercises. The therapist instructs, motivates, and assists patients and caregivers in improving functional mobility and activities of daily living. This role provides clinical supervision and oversight of Physical Therapist Assistants and Home Health Aides, ensures appropriate use of adaptive equipment, and maintains effective communication with the interdisciplinary care team.
