$45•50/hr Flexible scheduling to fit your lifestyle Opportunities across our facility network Employee appreciation events and incentives Position Summary We are seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered therapy services on a PRN basis. This role focuses on helping residents improve mobility, strength, and independence while working as part of an interdisciplinary care team. Key Responsibilities Evaluate residents and develop individualized treatment plans Provide skilled therapy services per physician orders and care plans Document evaluations, treatments, and progress notes in compliance with regulations Communicate effectively with physicians, nursing staff, and care team members Supervise PTAs, aides, and students in accordance with state practice acts Participate in care planning and interdisciplinary meetings Educate residents, families, and caregivers on therapy goals and discharge plans Conduct staff training and in-service education Maintain compliance with all state, federal, and payer guidelines Qualifications Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred) Active South Carolina Physical Therapist license (in good standing) Experience in skilled nursing or rehabilitation setting preferred Familiarity with PointClickCare and Casamba preferred Strong clinical, documentation, and communication skills We are an equal opportunity employer.
