$45-$50/hr Flexible PRN scheduling Reliable hours available PRN opportunities within the largest post-acute care network in South Carolina Supportive leadership team and collaborative therapy department Position Summary We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehabilitation team on a PRN basis. In this role, you will evaluate and treat residents, develop individualized treatment plans, collaborate with the interdisciplinary care team, and help residents achieve their highest level of mobility, function, and independence. Key Responsibilities Evaluate residents within 24 hours, or one business day, of physician referral whenever possible Develop individualized treatment plans and obtain physician approval as required Provide physical therapy treatments in accordance with physician orders and established goals Monitor resident progress and modify treatment plans as appropriate Assist the nursing department with training Restorative Aides Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state practice regulations Communicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team members Participate in care conferences, utilization review meetings, rehabilitation meetings, and discharge planning activities Complete evaluations, treatment notes, progress notes, recertifications, and other required documentation accurately and timely Educate residents, families, and caregivers on therapy goals, home exercise programs, safety techniques, and discharge recommendations Recommend and facilitate acquisition of durable medical equipment such as walkers, canes, braces, grab bars, and elevated toilet seats Report equipment concerns and assist in maintaining a safe therapy environment Conduct in-service education and training programs for facility staff as needed Supervisory Responsibilities Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policies Qualifications Education & Experience Bachelor's degree in Physical Therapy required Master's or Doctorate degree in Physical Therapy preferred Experience in skilled nursing, rehabilitation, or long-term care preferred Experience with PointClickCare (PCC) and Casamba preferred Strong clinical, communication, and documentation skills Licenses & Certifications Current South Carolina Physical Therapist license in good standing Must maintain all required continuing education requirements Must remain in good standing with the applicable State Board What Makes You a Great Fit Passion for helping residents regain mobility and independence Strong clinical judgment and problem-solving skills Excellent communication and interpersonal abilities Dependable and flexible Team-oriented and resident-focused Commitment to delivering exceptional therapy outcomes We are an equal opportunity employer.
