Delivers treatment based on a written plan of care, monitors progress, and adjusts interventions as needed. Responsible for discharge planning, patient and family education, and documenting care in medical records. May supervise clinical students, support staff, and volunteers, fostering a caring environment that promotes health and healing. Performs additional duties as assigned. Responsibilities Evaluates patients using subjective and objective data. Develops and implements a written plan of care. Provides treatment aligned with established goals. Documents interventions and patient progress accurately. Reassesses patient status and modifies care plans accordingly. Contributes to discharge planning and community resource referrals. Supports staff and student development. Participates in initiatives to enhance healthcare delivery. Completes assigned goals effectively. Experience Entry level required. Skills Basic skills including keyboarding, mouse use, and site navigation. Licensure Must obtain BLS certification within 14 days of hire.
