Job Overview We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Duties Assess patients' physical conditions by reviewing medical histories and conducting comprehensive evaluations of movement, strength, and function. Develop and implement individualized care plans that align with clinical best practices and patient goals. Administer therapeutic interventions such as manual therapy, trigger point therapy, therapeutic exercises, and modalities to promote healing and mobility. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare team members to coordinate patient care strategies. Document all treatment activities accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Educate patients and their families on rehabilitation exercises, injury prevention, and home care routines to support ongoing recovery. Manage discharge planning processes by preparing patients for transition from inpatient or outpatient settings to home or community care environments. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units, or home health environments. Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology. Demonstrated ability to develop effective treatment plans tailored to individual patient needs across diverse populations including geriatric and pediatric patients. Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and care coordination. Familiarity with outpatient rehabilitation center procedures and inpatient hospital protocols is highly desirable. Excellent client communication skills with the ability to motivate patients and foster a positive therapeutic environment. Knowledge of discharge planning procedures and experience working with acute care settings is a plus.
