Location:
Dallas Service Area, Texas Schedule:
Monday through Friday, 8:00 AM to 4:30
PM On-Call Requirement:
One weekend per month
Productivity Expectation:
30 points per week

Yes, between patient visits (from first patient to last patient) Position Overview We are seeking a compassionate and experienced Home Health Physical Therapist (PT) to join our growing team serving patients throughout the Dallas service area. The Physical Therapist will provide skilled therapy services in the home setting, helping patients regain mobility, improve function, and achieve their highest level of independence. This position offers a consistent weekday schedule, supportive clinical team environment, and the opportunity to build meaningful relationships with patients while delivering high-quality home health care. Key Responsibilities Conduct comprehensive physical therapy evaluations and assessments in patients' homes Complete and document OASIS assessments accurately and within required timeframes Provide skilled physical therapy treatment through subsequent and follow-up visits Develop individualized treatment plans based on patient needs and goals Educate patients, caregivers, and family members on therapy techniques, home exercise programs, and safety measures Collaborate with nurses, physicians, and interdisciplinary team members to ensure coordinated patient care Maintain timely, accurate, and compliant clinical documentation Monitor patient progress and modify treatment plans as appropriate Participate in weekend on-call rotation (one weekend per month) Required Qualifications Current Physical Therapist (PT) license in the State of Texas Minimum of 1 year of home health experience Experience completing OASIS assessments Proficiency with Homecare Homebase (HCHB) Strong clinical assessment and documentation skills Valid driver's license and reliable transportation Preferred Qualifications Candidates will stand out if they possess: Experience working with post-operative orthopedic patients Strong time management and organizational skills Excellent communication and patient education abilities Ability to work independently while maintaining strong collaboration with the clinical team Compensation & Benefits Competitive compensation package Mileage reimbursement between patient visits Consistent Monday through Friday schedule Opportunity to work autonomously while making a meaningful impact in patients' lives Dress Code Navy scrubs
