(PT) - HOME HEALTH Houston, TX & Surrounding Areas We are currently seeking a licensed Physical Therapist (PT) to join our home health team and provide quality, patient-centered care to individuals in the comfort of their own homes.Position Highlights:

Perform patient evaluations and develop individualized treatment plans Provide skilled physical therapy services in patients' homes Monitor patient progress and adjust treatment plans as needed Educate patients and caregivers on exercises and home programs Maintain accurate and timely clinical documentation Collaborate with physicians and other members of the healthcare teamRequirements:
