Job Description
Job Overview Join our dynamic healthcare team as a Home Health Physical Therapist, where you will play a vital role in delivering personalized rehabilitation services directly to patients in their homes. This energetic and rewarding position offers the opportunity to make a meaningful difference in the lives of diverse patient populations, including geriatrics, and individuals recovering from surgery or injury. You will assess patient needs, develop tailored treatment plans, and collaborate with a multidisciplinary team to promote optimal recovery and independence. Your expertise in physiotherapy and passion for patient care will drive positive outcomes and foster trust within the community. Duties Conduct comprehensive assessments of patients' physical abilities, mobility, and functional limitations within their home environment Develop and implement individualized treatment plans that align with clinical best practices and patient goals Perform therapeutic interventions such as manual therapy, trigger point therapy, orthopedics, neurology, and geriatrics-focused techniques to improve mobility and reduce pain Coordinate discharge planning with healthcare professionals to ensure seamless transition of care and continued recovery at home or in outpatient settings Maintain accurate and detailed clinical documentation adhering to documentation standards and electronic health records (EHR) management protocols Collaborate effectively with physicians, nurses, caregivers, and other healthcare providers to optimize patient outcomes Educate patients and their families on physiotherapy exercises, safety measures, and community resources to support ongoing rehabilitation Qualifications Valid licensure or certification as a licensed Physical Therapist with practicing in a physical therapy role Strong knowledge of physiology, anatomy, medical terminology, and treatment modalities relevant to orthopedics, neurology, geriatrics, pediatrics, and outpatient rehabilitation Experience with clinical documentation standards and EMR (Electronic Medical Records) systems for accurate record keeping Demonstrated ability to develop effective treatment plans tailored to individual patient needs Excellent client communication skills with a compassionate approach to patient care Familiarity with discharge planning processes and working with geriatric population is highly desirable Join us in delivering compassionate home health services that empower patients to regain independence!
