Position Summary We are seeking a patient-focused Physical Therapist with strong clinical skills and excellent customer service to join our dedicated team. This role plays a key part in improving resident mobility, independence, and overall quality of life. Key Responsibilities Conduct evaluations by reviewing medical histories, therapy referrals, and discussing goals with residents. Diagnose movement dysfunction through observation and functional assessment. Develop individualized treatment plans tailored to patient/resident needs. Instruct residents on therapeutic exercises, functional mobility, and strengthening programs. Provide manual therapy techniques, as clinically appropriate, to support healing and reduce pain. Document evaluations, treatments, progress notes, and care plans accurately and timely. Educate residents and families on home exercise programs, safety needs, and the recovery process. Collaborate with interdisciplinary team members to ensure coordinated, resident-centered care. Required Qualifications Graduate degree in Physical Therapy (DPT required) Current and active state licensure as a Physical Therapist . Excellent communication, listening, and interpersonal skills. Strong organizational, clinical reasoning, and problem-solving abilities. Ability to multitask and manage a diverse caseload. Flexibility to work weekends as needed. Preferred Qualifications Experience working in long-term care, skilled nursing, or rehabilitation. Familiarity with Net Health software.
