Webster, TX Job Details Contract $55 - $80 an hour 18 hours ago Benefits Flexible schedule Qualifications Inpatient experience Outpatient experience Physical Therapy License Patient interaction Orthopedics Medical terminology Full Job Description Overview We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will possess a strong understanding of medical terminology and have experience in various settings. This role is essential in helping patients regain mobility and improve their quality of life through tailored rehabilitation programs. This is a contract/PRN position and we are looking for a Physical Therapist who is willing to evaluate clients in South East Houston, TX (Webster), we have 3 clients that are under our care and we are needing a replacement PT. We only need an evaluator that will come once a month to evaluate and update plan of care since we have a PTA that will perform treatment/follow up sessions. This is a home health setting and so you will be evaluating/treating clients in personal homes. You will be able to choose your own schedule and which clients you would like to pick up based on location. There is no minimum or maximum amount of clients that you have to have to be in the Adapt & Go Therapy system. You will be reimbursed for any far travel. Duties Conduct comprehensive assessments of patients' physical abilities and limitations. Develop individualized treatment plans based on patient evaluations and goals. Implement therapeutic exercises and modalities to enhance recovery. Educate patients and their families on treatment plans, home exercises, and injury prevention strategies. Collaborate with other healthcare professionals to ensure coordinated patient care. Monitor patient progress and adjust treatment plans as necessary. Assist with discharge planning to facilitate smooth transitions from inpatient to outpatient care or home community care. Maintain accurate documentation of patient treatments, progress notes, and discharge summaries. Experience Knowledge of medical terminology is essential for effective communication within the healthcare team. Experience in orthopedics and physiology knowledge is highly desirable. Familiarity with patient care practices in both inpatient and outpatient settings is preferred. Previous experience in home health or community-based care will be an advantage. Understanding of discharge planning processes to support patient transitions is beneficial.
