Key Responsibilities Conduct thorough initial and ongoing assessments to evaluate patient mobility, pain levels, strength, and overall functionality, adapting strategies as necessary based on progress. Develop and implement comprehensive, individualized treatment plans that align with the goals set by the residents and their physicians, ensuring a tailored therapeutic approach. Deliver evidence-based therapy sessions focused on therapeutic exercises, techniques to enhance mobility, and strategies to reduce pain while preventing future injuries. Educate residents, families, and caregivers on at-home therapeutic exercises and wellness practices to promote continued progress and independence. Work collaboratively with nursing staff, occupational therapists, and wellness teams to develop interdisciplinary care plans that address all facets of patient health. Maintain accurate and timely documentation of patient progress and treatment plans in compliance with Medicare and state regulations, ensuring standards of care are met and maintained. Contribute positively to a patient-centric environment that respects the dignity and individuality of our clients, promoting a culture of care and compassion. Patient Population / Client Focus As a Physical Therapist at Resurge Rehab & Therapy, you will primarily serve a diverse clientele in home care settings, including elderly residents and individuals recovering from surgeries, injuries, or illnesses. Your focus will be on helping residents maintain independence, build strength, and improve their overall mobility. You will work specifically with a population that values personalized care within their home environments, allowing for direct and meaningful engagement that promotes better outcomes. Qualifications A current and valid Physical Therapist license in Virginia, or eligibility for licensure. A degree from an accredited Physical Therapy program (DPT or equivalent). CPR/BLS certification or the willingness to obtain certification upon hire. Exceptional communication, organizational, and empathy skills essential for effective patient interaction. A strong ability to work independently while being a productive member of a collaborative care team. Preferred Experience Experience in geriatrics or long-term care is preferred but not mandatory. A background in home health therapy practices will be an advantage. Familiarity with electronic health records (EHR) systems to maintain patient documentation efficiently. Compensation and Benefits Resurge Rehab & Therapy offers a competitive salary ranging from $90,000 to $100,000 annually, commensurate with experience and skills.
