Overview:
Warm Hearth at Home is looking to expand our team of Home Health physical therapists. Warm Hearth Village is a non-profit community of seniors that has been serving Blacksburg and the surrounding New River Valley for over 50 years. Our mission is to foster a person-centered approach to aging in an inclusive environment that redefines retirement, and to nurture a transformative culture where people live and work in community, knowing their contributions are essential to our success. We are currently hiring a regular PRN Physical Therapist with the possibility of transitioning to Part time/full time as our census grows. Our Home Health agency is staffed fully in-house with a small, tight-knit administrative staff and HR department. We have experienced steady growth over the past two years with plans of continued expansion to increase our capacity to serve our community.
Responsibilities:
As a Home Health Physical Therapist, You will: Evaluate and Assess patients in the home setting and closely collaborate with PTAs to ensure individualized and effective intervention Establish measurable and meaningful goals with regular reassessment to ensure progress and to maximize effectiveness of therapy Maintain clear and effective communication regarding patient care with support staff, physicians, and family members to ensure efficient care Complete documentation timely, accurately, and completely
Qualifications:
Graduate of a Physical Therapist Education program approved by CAPTE Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire Drivers license and proof of auto insurance Experience in home health setting is preferred Ability and confidence to recognize red flags and/or medical emergencies in the home, and how to effectively triage
