Work Environment This position is primarily onsite, where you will deliver hands-on therapy to patients in their homes. Working in a home care setting allows for greater flexibility in treatment and gives you the opportunity to understand patients in their own environments, which is crucial for effective rehabilitation. Key Responsibilities Conduct comprehensive patient evaluations, including clinical assessments based on the patient's medical history, functional capabilities, and specific rehabilitation needs. Develop and implement customized treatment plans that meet the individual goals of your patients. Your treatment approach will be grounded in evidence-based methodologies, leveraging your clinical expertise to apply the most effective therapeutic techniques with the goal of maximizing patient outcomes. Maintain accurate and timely documentation, reflecting each patient's course of care, from initial evaluations to discharge summaries. This documentation includes establishing baseline goals, ongoing progress notes, and post-treatment evaluations. Engage in consultative discussions with other healthcare professionals, when necessary, ensuring that any changes in patient condition are promptly recognized, documented, and addressed as per the plan of care. Participate actively in discharge planning to ensure smooth transitions for patients moving from therapy to home-based independence, including coordination of follow-up care. Aim for clear communication with patients, their families, and multidisciplinary team members to foster a supportive recovery environment and align treatment goals with patient needs. Engage in ongoing professional development by participating in facility meetings, training sessions and compliance with state and federal regulations, ensuring continuous quality improvement in patient care. Patient Population / Client Focus You will primarily work with orthopedic cases, treating patients recovering from surgeries, trauma, and various musculoskeletal disorders. Your expertise will empower patients of all ages to regain mobility, reduce pain, and lead more active lifestyles. Working closely with each patient in their home environment allows for bespoke strategies that consider their daily routines, which is essential in creating effective rehabilitation plans. Qualifications A Doctorate Degree in Physical Therapy (DPT) is required. Current licensure to practice as a Physical Therapist in Virginia is a must. Certification in Basic Life Support (BLS) from the American Heart Association is required prior to start. Preferred Experience A minimum of 2 years of clinical experience, preferably within an orthopedic or rehabilitative setting, is required. Proficiency in applying evidence-based therapy techniques and effective communication skills is essential for success in this role. Compensation and Benefits The salary range for this position is between USD 100,000 and 120,000 per year, commensurate with experience and qualifications.
