Responsibilities:

Conduct comprehensive physical therapy assessments and develop personalized treatment plans to achieve patients' goals and improve their quality of life Provide one-on-one physical therapy interventions, including exercises, modalities, and education to patients and their caregivers Collaborate with our interdisciplinary team, including physicians, nurses, and social workers, to ensure comprehensive care plans Maintain accurate and timely documentation of patient progress, treatment plans, and billing information Develop and implement strategies to prevent hospital readmissions and improve patient outcomes Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy About the

Requirements:
