Work Environment This position requires working onsite as a home care Physical Therapist, which offers the unique opportunity to meet patients in their home environments. You will work a standard 40-hour week with some variability over weekends to accommodate patient schedules. Our interdisciplinary team values open communication and collaboration as we support each other in delivering the highest quality of care. Key Responsibilities Conduct comprehensive assessments to examine, evaluate, and develop individualized treatment plans for patients experiencing functional limitations or disabilities. Provide expert consultation to healthcare providers, ensuring seamless continuity of care for patients. Adhere to high standards of customer service when interacting with patients across various age groups and cultures. Educate patients and their families about treatment protocols, participating actively in discharge planning. Document evaluations, treatment plans, and progress notes accurately in compliance with state regulations and professional standards. Collaborate effectively with an interdisciplinary team, including physicians, to update patient care strategies as necessary. Participate in community outreach initiatives and maintain involvement in continuing education programs to stay updated on best practices and innovations in physical therapy. Supervise and mentor assigned support staff, ensuring the appropriate allocation of resources. Manage department records and optimize the use of tools and technologies to streamline patient care and documentation processes. Patient Population / Client Focus Your role will primarily involve working with a diverse patient population, including elderly individuals, post-operative patients, and those managing chronic conditions in their homes. Compensation and Benefits This full-time Physical Therapist position offers a competitive annual salary ranging from
USD 100,000
to 120,000, commensurate with experience.
