Description:
Position Overview We are seeking a dedicated and skilled Orthopedic Physical Therapist to join our esteemed team at Patient Plus Therapy in Leesburg, Virginia. The Orthopedic Physical Therapist will play a pivotal role in assessing patient needs, developing treatment plans, and implementing evidence-based therapies that cater to individual health goals. Work Environment This position is onsite, primarily based in patients' homes in the varying communities of Leesburg, Virginia. Our physical therapy team operates with flexibility, addressing the unique needs of each patient within their familiar surroundings. Working in home care helps enhance the therapeutic relationship and allows for personalized treatment that adapts to the patient's daily life and environment. Key Responsibilities Conduct comprehensive evaluations of patients, including gathering relevant medical history and performing specific assessments to determine the suitability of treatment protocols. Articulate evaluation findings and collaboratively develop treatment plans with patients and their families, setting achievable goals tailored to individual circumstances. Exhibit expertise in executing treatment protocols rooted in evaluation outcomes, addressing patient needs in a dynamic healthcare landscape. Engage in interprofessional collaboration, consulting with healthcare colleagues as necessary, and modify care plans in response to changes in patient conditions. Manage and execute discharge planning effectively, ensuring comprehensive follow-up care that adheres to best practices. Maintain precise documentation of patient care activities, encapsulating the course of treatment, daily encounters, progress summaries, and discharge evaluations, in compliance with healthcare regulations. Demonstrate strong time management skills while consistently achieving productivity benchmarks, attending meetings punctually, and participating in both hospital and community outreach initiatives. Contribute actively to the attainment of departmental objectives, ensuring compliance with state and national accreditation standards. Take initiative in pursuing continuing education opportunities that foster professional growth and skill enhancement, serving occasionally as a clinical instructor for students. Advocate for an exceptional patient experience by being adaptable to patient needs and promoting healthcare access initiatives. Patient Population / Client Focus The Orthopedic Physical Therapist will primarily serve individuals recovering from orthopedic conditions, surgeries, and sports-related injuries. This patient demographic often requires tailored rehabilitation exercises, education on injury prevention, and strategic planning to regain optimal function, allowing them to return to their daily activities. The care environment promotes both physical and emotional support, crucial for patient recovery. Qualifications A Doctorate of Physical Therapy (DPT) from a program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE). Current state licensure as a Doctor of Physical Therapy in accordance with the regulations of Virginia. A minimum of two years of clinical experience in physical therapy, specifically focusing on orthopedic rehabilitation. A valid Cardiopulmonary Resuscitation (CPR) certification. Preferred Experience Experience with electronic medical record (EMR) systems to streamline patient documentation processes. Prior exposure to home care environments is an added advantage, underscoring the ability to adapt therapeutic practices to patients' individual home settings. Familiarity with community health initiatives and participation in health promotion activities can enhance practice knowledge. Compensation and Benefits Patient Plus Therapy offers a competitive salary ranging from $100,000 to $120,000 per year, alongside a comprehensive benefits package designed to support your health and well-being.
