Acute Care Physical Therapist - Join Our Hospital Rehab Team - Weekend Rotation.
Shift:

The Acute Care Physical Therapist provides physical therapy evaluation and treatment to patients with musculoskeletal and/or neurological impairments. The Physical Therapist is also responsible for the monitoring and documentation of patient status, treatment, and response.
Responsibilities:
Performs physical therapy evaluations and develops a plan of care that is appropriate for the diagnosis and age of patient. Performs physical therapy patient treatment as per the plan of care and modifies treatment plan based on ongoing assessment of patient condition. Demonstrates an understanding of the operation and indications for commonly used modalities and equipment. Documents evaluations, daily notes, progress notes, and discharge notes in a timely manner; therapy charges are supported by this documentation. Communicates and documents necessary information appropriately with patients, families, physicians, and interdisciplinary team members.
