Full-Time or Part-Time (2 days per week / 12 visit per week)
Compensation:

What You'll Do As a Physical Therapist, you'll practice at the top of your license while helping patients recover following surgery, illness, injury, or hospitalization. You'll evaluate patients in their home environment, identify barriers to independence, and develop personalized plans of care that improve mobility, strength, balance, and overall quality of life. You'll collaborate closely with Physical Therapist Assistants and the interdisciplinary team while helping patients achieve meaningful goals that matter to them.
Responsibilities include:
In-home evaluations and treatment visits. Admissions and start-of-care assessments. Development and modification of individualized treatment plans. Supervision and collaboration with PTAs. Coordination of care with nurses, physicians, and other team members. Monitoring progress and adjusting treatment plans as needed. Timely completion of documentation and OASIS assessments when applicable. Qualifications Current Minnesota Physical Therapist License Required Home health experience preferred. OASIS experience preferred. Ability to independently manage a territory and caseload. Strong clinical judgment and communication skills. Valid driver's license and reliable transportation.
