Our skilled nursing and post-acute rehabilitation center is dedicated to helping patients regain strength, mobility, and independence following surgery, illness, or injury. As a valued member of our interdisciplinary team, you will work alongside Physical Therapists, Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and case managers to deliver outstanding patient-centered care. Position Summary Under the supervision of a licensed Physical Therapist, the Physical Therapist Assistant will implement individualized treatment plans designed to restore function, improve mobility, reduce pain, and maximize each patient's independence. This position provides the opportunity to work with a diverse geriatric and medically complex population in a collaborative Skilled Nursing Facility (SNF) setting. Responsibilities Implement individualized treatment plans developed by the supervising Physical Therapist. Provide therapeutic exercise, gait training, balance training, transfer training, functional mobility training, and manual therapy techniques. Monitor patient progress and communicate treatment outcomes with the supervising therapist and interdisciplinary care team. Document patient treatments accurately and timely in the Electronic Medical Record (EMR/EHR). Educate patients and caregivers on home exercise programs, safety strategies, and techniques to promote continued recovery. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to deliver coordinated, patient-centered care. Assist with discharge planning to support successful transitions home or to the next level of care. Maintain a safe, clean, and organized treatment environment. Ensure compliance with Medicare guidelines, HIPAA regulations, state practice standards, and company policies. QualificationsRequired Graduate of an accredited Physical Therapist Assistant (PTA) program. Current Arizona Physical Therapist Assistant license in good standing. Current CPR/BLS certification or ability to obtain upon hire. Strong communication, organization, and clinical documentation skills. Ability to work effectively within a collaborative interdisciplinary healthcare team. Preferred Experience in a Skilled Nursing Facility (SNF), post-acute rehabilitation, inpatient rehabilitation, or long-term care setting. Experience treating orthopedic, neurological, medically complex, and geriatric patients. Familiarity with Medicare documentation, care planning, discharge planning, and EMR/EHR systems. Skills Deliver patient-centered rehabilitation focused on restoring strength, mobility, balance, and independence. Implement evidence-based treatment interventions under the direction of a licensed Physical Therapist. Perform gait training, therapeutic exercise, balance activities, transfer training, and functional mobility interventions. Collaborate with nursing, occupational therapy, speech therapy, physicians, social services, and case management to achieve optimal patient outcomes. Support safe transitions from hospital to home through individualized discharge planning and caregiver education. Build trusting relationships with patients and families while providing compassionate, high-quality care. Maintain accurate and timely clinical documentation in accordance with Medicare and facility standards. Adapt treatment approaches to meet the needs of medically complex patients in a fast-paced Skilled Nursing Facility environment.
