Job Description

South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to provide hands-on rehabilitative care in skilled nursing and post-acute settings across Hawaii. In this role, you will implement treatment plans under the direction of a Physical Therapist, focusing on improving mobility, strength, balance, and functional independence. You'll collaborate closely with an interdisciplinary rehab team, document patient progress, and educate patients and families. Responsibilities Implement physical therapy treatment plans under the supervision of a Physical Therapist Provide therapeutic exercises, gait and balance training, and functional mobility interventions Assist with transfers, bed mobility, and use of assistive devices to promote safety and independence Accurately document treatments, patient progress, and communication in the EMREducate patients and families on exercises, safety, and strategies to maintain functional gains Collaborate with PTs, OTs, nursing, and other team members on patient care plans Monitor patient response to treatment and report changes to the supervising PTMaintain a safe, clean treatment environment and proper equipment use Participate in interdisciplinary meetings and care conferences as needed Engage in ongoing professional development and training offered by the organization Required Skills Therapeutic exercise implementation Gait training Neuromuscular re-education Balance and fall prevention programs Functional mobility training Transfer and bed mobility techniques Use of rehab equipment and modalities Electronic medical record documentation Patient and family education Interdisciplinary team collaboration
