Our administrative team provides support with referral coordination, scheduling communication, documentation requirements, and other day-to-day needs so clinicians can focus on providing excellent patient care. New graduates and clinicians new to home health are welcome to apply. Current Coverage Areas LJM Therapy is currently expanding throughout Los Angeles County and surrounding Southern California communities , with a focus on: Downey, Bellflower, Norwalk, Lakewood, Cerritos, Long Beach, North Long Beach, Paramount, South Gate, Huntington Park, Bell, Bell Gardens, Cudahy, Maywood, Commerce, Compton, Lynwood, Gardena, Carson, and surrounding communities. Additional coverage opportunities may become available as our referral network expands. What You'll Do Provide skilled Physical Therapist Assistant services in patients' homes under the direction and supervision of a licensed Physical Therapist. Follow the established PT plan of care and physician orders. Provide therapeutic exercise, gait training, transfer training, balance training, strengthening, patient/caregiver education, and other appropriate interventions. Monitor patient progress and communicate significant changes to the supervising Physical Therapist. Complete required clinical documentation accurately and within established timeframes. Maintain patient safety and provide professional, compassionate care. Follow applicable California laws, payer requirements, agency policies, and professional standards. What We're Looking For Current California Physical Therapist Assistant (PTA) License Valid driver's license Reliable transportation Ability and willingness to travel to patients' homes within selected coverage areas Strong communication and documentation skills Dependable, professional, and committed to excellent patient care Home health experience preferred but not required Job Types Independent Contractor Opportunities Full-time, Part-time, Contract, Per Diem, and PRN opportunities available.
