Full Time/Part Time Responsibilities:

Assess and Evaluate:

Conduct comprehensive assessments to determine residents' physical abilities, functional limitations, and rehabilitation goals.

Treatment Planning:

Develop individualized treatment plans based on assessments and residents' specific needs, focusing on improving mobility, strength, balance, coordination, and overall functional independence.

Therapy Implementation:

Administer appropriate therapeutic interventions, exercises, manual therapy techniques, and modalities to enhance residents' physical function, reduce pain, and promote optimal recovery.

Monitoring and Progress Evaluation:

Regularly evaluate residents' progress, adjust treatment plans as needed, and document outcomes accurately to track improvement and ensure effective care.

Patient Education:

Educate residents and their families on therapeutic techniques, home exercise programs, and strategies to maintain progress and prevent further injury or decline.

Collaborative Approach:

Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate comprehensive care plans and ensure the holistic well-being of residents.

Compliance and Documentation:

Maintain accurate and up-to-date documentation of all evaluations, treatments, and progress notes in compliance with facility and regulatory guidelines.

Safety and Infection Control:

Follow infection control protocols and maintain a safe environment for residents and staff during therapy sessions.

Continuous Learning:

Stay updated with advancements in physical therapy techniques, evidence-based practices, and industry standards through professional development opportunities and participation in relevant workshops or conferences.

Quality Improvement:

Contribute to the facility's quality improvement initiatives by providing input, participating in team meetings, and implementing best practices to enhance patient outcomes and overall service excellence.

Qualifications:

Education:

Hold a Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.

Licensure:

Possess a valid state license or be eligible for licensure as a Physical Therapist Assistant in the state where the Skilled Nursing Facility is located.

Experience:

Prior experience working as a Physical Therapist Assistant in a similar setting, such as a skilled nursing facility or rehabilitation center, is preferred.

Clinical Skills:

Proficient in conducting assessments, creating customized treatment plans, and implementing evidence-based physical therapy techniques. Experience with various therapeutic modalities, such as ultrasound, electrical stimulation, and therapeutic exercises, is desired.

Communication and Interpersonal Skills:

Excellent verbal and written communication skills, with the ability to effectively interact with residents, families, and interdisciplinary team members.

Empathy and Compassion:

Demonstrated empathy, patience, and compassion in providing care to older adults with diverse physical conditions and functional limitations.

Problem-Solving Abilities:

Strong critical thinking and problem-solving skills to address challenges in therapy and develop innovative solutions.

Time Management:
