Job Description

South Pacific Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide direct patient care under the supervision of a Physical Therapist in skilled nursing and post-acute settings. Responsibilities include implementing treatment plans, documenting progress, and collaborating with an interdisciplinary team to promote functional recovery and quality of life. Responsibilities Implement PT treatment plans under direction of supervising Physical Therapist Provide one-on-one therapeutic interventions to improve mobility, strength, and function Assist with gait training, transfers, and ADL-focused activities in skilled nursing and post-acute settings Accurately document treatments, patient response, and progress in EMR systems Collaborate with PTs, nurses, and interdisciplinary team to support resident-centered goals Educate patients, families, and caregivers on exercises, safety, and functional strategies Monitor patient status and promptly report changes to supervising PT and nursing staff Contribute to care plan meetings and outcome-focused rehab programs Adhere to ethical, regulatory, and documentation standards for therapy services Participate in ongoing education, competency development, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training Use of therapeutic modalities (e.g., heat, cold, electrical stimulation per policy) Patient transfer and safe handling techniques Treatment documentation and EMR use Care planning in skilled nursing/post-acute settings Patient and family education Interdisciplinary team collaboration Outcome tracking and progress assessment
