South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, one-on-one rehab services in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you'll implement care plans, guide therapeutic exercises, document progress, and support safe mobility and functional independence. Responsibilities Provide direct physical therapy interventions under supervision of a Physical Therapist Implement and progress treatment plans focused on mobility, strength, balance, and function Assist with transfer training, gait training, and use of assistive devices Observe, measure, and document patient status and response to treatment in the EMRCommunicate patient progress and concerns to supervising PT and interdisciplinary team Educate patients, families, and caregivers on exercises, safety, and functional strategies Follow all regulatory, documentation, and ethical practice standards Contribute to discharge planning and recommendations for continued services or equipment Participate in team meetings, in-services, and ongoing clinical education Maintain a safe, clean, and patient-centered treatment environment in the facility Required Skills Therapeutic exercise implementation Gait training and transfer techniques Functional mobility training Use of assistive devices and DMEPatient and family education Clinical documentation/EMR use Knowledge of SNF/post-acute regulations Vital sign monitoring and safety awareness Treatment progression and modification Interdisciplinary team communication
