Job Description

Tender Touch Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide high-quality therapy services to seniors in skilled nursing and long-term care settings, implementing treatment plans under the direction of a Physical Therapist. You will help improve residents' function, independence, and quality of life while collaborating with an interdisciplinary team. Responsibilities Provide one-on-one physical therapy interventions under PT supervision for seniors in skilled nursing/long-term care settings Implement individualized treatment plans focused on functional mobility, strength, balance, and safety Assist with gait training, transfer training, therapeutic exercises, and use of assistive devices Accurately document treatments, progress notes, and functional outcomes in the EMR in a timely manner Collaborate closely with PTs, nursing, and the interdisciplinary team to optimize patient outcomes and discharge planning Educate patients, families, and caregivers on home exercise programs, safety strategies, and functional independence Monitor patient response to treatment, report changes to supervising PT, and help adjust plans as needed Contribute to a supportive, patient-centered environment that emphasizes dignity, compassion, and respect for seniors Required Skills Therapeutic exercise implementation Gait and transfer training Balance and fall prevention techniques Modalities application (e.g., heat, cold, e-stim, ultrasound) Documentation in EMR systems Functional mobility and ADL training Patient and family education Interdisciplinary care coordination Infection control and safety procedures Pain management techniques
