Job Description
VitalCaring seeks a compassionate Physical Therapy Assistant to provide home-based care for home health and hospice patients. Working under the direction of a licensed Physical Therapist, you will deliver therapeutic exercises, mobility training, and home safety interventions that support recovery, comfort, and independence. You'll travel to patient homes, document care in our EHR, and collaborate with an interdisciplinary team. Responsibilities Provide physical therapy interventions to home health and hospice patients under direction of a licensed Physical Therapist Assist with patient assessments, care planning, and progression documentation in accordance with clinical standards Educate patients and caregivers on exercises, mobility techniques, and safe use of assistive devices in the home Travel to patient homes, ensuring timely, compassionate, and professional care delivery across assigned territory Accurately document visits, patient progress, and outcomes in the electronic health record Collaborate with therapists, nurses, and interdisciplinary team members to coordinate patient-centered care plans Monitor patient response to treatment and promptly report changes in condition to supervising Physical Therapist or clinical leadership Support discharge planning, functional goal attainment, and strategies to improve safety and independence at home Required Skills Therapeutic exercise implementation Gait training and transfer techniques Home safety and fall-risk assessment Patient and caregiver education Electronic health record (EHR) documentation Pain management modalities within PTA scope Vital sign monitoring and basic clinical assessment Care planning support and progress reporting Time management and route planning for home visits Interdisciplinary team collaboration
