Job Description

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based care to seniors in skilled nursing and long-term care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans focused on mobility, strength, balance, and functional independence. Responsibilities include delivering therapeutic interventions, documenting outcomes, educating patients and families, and collaborating with an interdisciplinary team. Responsibilities Deliver physical therapy interventions under PT supervision to support strength, balance, and mobility for senior patients Assist with evaluations by performing delegated tests and reporting patient status and response to treatment Implement individualized treatment plans focused on function, independence, and safety in skilled nursing and long-term care settings Accurately document treatments, progress notes, and functional outcomes in the EMR in a timely manner Educate patients, families, and caregivers on home exercises, safety, and functional strategies Collaborate with an interdisciplinary team, including nursing and rehab colleagues, to coordinate patient-centered care Maintain compliance with state practice acts, regulatory standards, and company policies Support a compassionate, dignity-focused environment that respects seniors and promotes quality of life Participate in professional development, mentorship, and clinical training opportunities Contribute to a culture of teamwork, communication, and clinical excellence Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Therapeutic modalities application Treatment documentation and EMR use Care planning in SNF/LTC settings Patient and family education Interdisciplinary team collaboration Vital signs monitoring and safety awareness Infection control and fall prevention
