- Mission-driven nonprofit organization committed to excellence, compassion, and innovation Position Summary The Physical Therapist Assistant will implement an individual patient therapy care plan that has been prepared by the Physical Therapist and work under the supervision of the Physical Therapist.
Reporting to the Supervisor, Rehab Services, your essential responsibilities include, but may not be limited to: How You'll Make an Impact
- Perform routine strengthening exercises and therapeutic exercises as outlined by the lead PT.
- Assist in teaching exercise programs to patient and family.
- Consult with therapist regarding the outcome of home visits and report findings in written forms.
- Submit written daily notes as required.
- Observe and record activities and findings in the record.
- Instruct other health team personnel and family members in certain aspects of physical therapy with which they work with the client.
- Participate with staff, client and physician in discharge activities and assists in completing the discharge summary.
- Communicate effectively with all those providing care.
- Performs other job duties as assigned.
