Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement treatment plans, guide therapeutic exercises, and document patient progress to improve mobility, function, and quality of life. You'll collaborate closely with an interdisciplinary team in a supportive, patient-centered culture that values integrity, respect, and clinical excellence. Responsibilities Provide therapy interventions under the supervision and direction of a licensed Physical Therapist Implement individualized treatment plans focused on functional mobility, strength, and balance Assist with gait training, therapeutic exercise, and use of assistive devices for seniors Accurately document treatments, patient responses, and progress in the medical record Collaborate with PTs, OTs, nursing, and other interdisciplinary team members on patient care Educate patients, families, and caregivers on home exercise programs and safety strategies Ensure adherence to all regulatory, documentation, and compliance standards Contribute to discharge planning and recommendations for continued care or equipment needs Participate in case conferences, in-services, and ongoing clinical education activities Promote a safe, respectful, and patient-centered environment consistent with company values Required Skills Therapeutic exercise implementation Gait training and balance rehabilitation Functional mobility and transfer training Use and adjustment of assistive devices Geriatric rehabilitation techniques Skilled nursing and post-acute care documentation Electronic medical record (EMR) proficiency Patient and family education for home programs Understanding of Medicare and rehab billing basics Infection control and safety procedures
